\"DoctorsI am proud to count myself among the <\/span>cadre<\/span> of physicians who incorporate healthcare informatics in their medical practice. <\/span>We may have downsized our clinical practice<\/span>s<\/span>, but we remain as strongly committed to the principles of the Hippocratic Oath <\/span>as we were when<\/span> we took <\/span>the oath<\/span> <\/span>in medical school. Our priority has <\/span>been <\/span>and always will be to put the health and safety of our patients front and center of everything we do in our profession. <\/span> <\/span><\/p>\n

I remember <\/span>how fervently we worked in the first decade of the 21<\/span>st<\/span> century to introduce computerized physician order entry (CPOE) into our systems. We <\/span>understood from the Crossing the Quality Chasm report (IOM, 2001)<\/span>, which <\/span>identifie<\/span>d<\/span> and recommend<\/span>ed<\/span> improvements <\/span>for<\/span> <\/span>US <\/span>healthcare<\/span>, <\/span>that CPOE <\/span>would make our patients safer<\/span>. <\/span>I also recall the disappointment we physician informaticists felt in the following<\/span> decade<\/span> <\/span>when it became clear that information technology<\/span> (IT)<\/span> was one of the contributing factors to physician burnout. IT bore the brunt of the blame<\/span>, sometimes unfairly,<\/span> for alienating physicians from their craft of medicine<\/span>. <\/span> <\/span> <\/span><\/p>\n

The turning point was marked by Bodenheimer and Sinsky, 2014, <\/span>who <\/span>explicit<\/span>ly<\/span> <\/span>add<\/span>ed<\/span> <\/span>a fourth ten<\/span>e<\/span>t<\/span>,<\/span> \u201c<\/span>improving the work life of healthcare providers<\/span>,<\/span>\u201d to the other three ten<\/span>e<\/span>ts of \u201c<\/span>enhancing patient experience, improving outcomes<\/span>,<\/span> and reducing costs\u201d to create the quadruple aim: a pivotal underpinning of a highly effective healthcare system. <\/span> <\/span><\/p>\n

Unfortunately, we continue to see an increasing number of reports that indicate rising percentages of physician burnout to this day. <\/span>In my opinion <\/span>this is <\/span>not a physician problem<\/span>,<\/span> but a systematic alienation that keeps physicians from doing what brings them a sense of satisfaction, well-being<\/span>,<\/span> and following their \u201ccalling.\u201d <\/span> <\/span><\/p>\n

Hartzband<\/span>, MD, and <\/span>Groopman<\/span>, MD, eloquently capture how the<\/span> COVID-19 pandemic offers insights into how we bring back a sense of joy <\/span>to<\/span> <\/span>the work of physicians (Physician Burnout, Interrupted, NEJM, June 2020)<\/span>. <\/span>As the<\/span> pandemic surged and uncertainty grew, physicians, nurses<\/span>,<\/span> and all healthcare workers became essential to the prevention, treatment<\/span>,<\/span> and survival of everyone across the globe<\/span>. <\/span>Clinicians worked urgently for excessively long hours under dire conditions, with <\/span>a <\/span>lack of protective equipment and <\/span>a <\/span>severe shortage of beds and ventilators<\/span>. <\/span> <\/span><\/p>\n

Systems and processes changed abruptly to allow physicians to deliver<\/span> expedited care in a more effective<\/span> <\/span>manner <\/span>with waivers<\/span> like <\/span>payment for virtual visits<\/span> and streamlined documentation<\/span>.<\/span> <\/span>We saw thousands of public displays of gratitude and appreciation for physicians, nurses<\/span>,<\/span> and all health workers across the globe. For the moment, clinicians are feeling that<\/span> inner satisfaction of helping and healing others that drew them to the profession in the first place.<\/span> <\/span> <\/span><\/p>\n

A new <\/span>U.S. <\/span>Health and Human Services (HHS)<\/span> <\/span>survey<\/span> of <\/span>3<\/span>2<\/span>0 <\/span>US <\/span>hospitals from February 22-26<\/span>, 202<\/span>1<\/span>,<\/span> highlights<\/span> the <\/span>massive<\/span> <\/span>toll a year of treating a global pandemic has taken on <\/span>the health system<\/span>,<\/span> <\/span>including the<\/span> <\/span>trauma<\/span> and<\/span> <\/span>stress <\/span>experience<\/span>d<\/span> by <\/span>medical staff <\/span>operating in “survival mode” for such a lengthy perio<\/span>d<\/span> and<\/span> <\/span>facing<\/span> <\/span>unprecedented numbers <\/span>of <\/span>deaths<\/span> every day<\/span>.<\/span> <\/span><\/p>\n

I am concerned<\/span> that as the pandemic is brought under control, and the health system overloads with patients who have put care on hold due to social distancing, we will see <\/span>another surge in physician burnout. <\/span>Silverman, MD, described <\/span>the pre-pandemic <\/span>frustration of physicians <\/span>in the New York Times: <\/span>\u201c<\/span>r<\/span>elentless reminders of tasks we haven\u2019t completed, supplications to correct our documentation for billers, and daily, jaundiced reminders: You are currently deficient<\/span>.\u201d<\/span> <\/span> <\/span><\/p>\n

We <\/span>must not allow this to happen<\/span>. W<\/span>e need to take a hard look at ourselves and make these <\/span>regulatory <\/span>waivers permanent<\/span>. <\/span>We need to implement health information technology that ensures good management practices<\/span>,<\/span> while still empowering physicians with a sense of autonomy, competence<\/span>,<\/span> and connectedness<\/span>. <\/span> <\/span><\/p>\n

Here are a few examples:<\/span> <\/span><\/p>\n

At the 2020 Healthcare Information and Management Systems Society (HIMSS) Global Health Conference & Exhibition, which was canceled due to the pandemic, Microsoft and Nuance<\/a><\/u> were poised to show a transformational digital ambient experience that automatically generated clinical documentation. This <\/span>technology <\/span>lets physicians spend more time with patients, while documenting information accurately and completely in much less time.<\/span> <\/span><\/div>\n

Virtual visits became a necessity <\/span>during<\/span> <\/span>the pandemic<\/span>,<\/span> and many hurdles were removed to make <\/span>them <\/span>easier for patients and physicians alike. If physicians are to provide high<\/span>–<\/span>quality care, <\/span>virtual <\/span>visits will need to be enhanced with technology that captures the clinical signs that physicians have <\/span>observed<\/span> for centuries.<\/span> <\/span>Remote <\/span>Internet of Things (<\/span>IoT<\/span>)<\/span> technology is ready to provide <\/span>information about <\/span>heart and respiratory rate<\/span>s<\/span>, blood pressure, movement analysis, auscultation<\/span>,<\/span> and much more. <\/span>Using <\/span>remote monitoring solutions<\/span> <\/span>can <\/span>help physicians <\/span>feel <\/span>more <\/span>competent and <\/span>co<\/span>nfident<\/span> and<\/span>, consequently,<\/span> less burned out.<\/span> <\/span> <\/span><\/p>\n

The pandemic has spotlighted the longstanding existence of health inequity, and physicians <\/span>can no longer ignore this blatant disparity<\/span>. <\/span>We need to root out systemic inequality so that we continue to feel the intrinsic motivation of our Hippocratic Oath<\/span>,<\/span> <\/span>of<\/span> treat<\/span>ing<\/span> everyone equally, regardless of their circumstances. I believe <\/span>aligning our practice with these ethics<\/span> is <\/span>vital<\/span> to reducing physician burnout. <\/span> <\/span><\/p>\n

We need clinical and operational analytics that can simultaneously measure <\/span>patient <\/span>outcomes and clinician experience. Microsoft has introduced<\/span> Viva<\/a>,<\/span> a new worker experience platform<\/span>, <\/span>that provides key insights for top-of-mind issues including well<\/span>–<\/span>being, burnout, agility<\/span>,<\/span> and customer focus. Incorporating these personal experience tools will <\/span>contribute to<\/span> reduc<\/span>ing<\/span> burnout in our health information technology environments.<\/span> <\/span><\/p>\n

On this National Doctors\u2019 Day 2021, let us pause for a moment to show our gratitude to physicians on the<\/span> frontlines of healthcare, for all that they do every day to keep us safe and healthy. Let us dedicate ourselves to expanding the use of health information technology and using it as a foundation for better healthcare and<\/span> as<\/span> a medium for delivering a joyful and satisfying experience for clinicians<\/span>,<\/span> and patients.<\/span><\/p>\n
